Here's most of them:
Call Forwarding: Activate: *72 + full number; Deactivate: *720 or *73
Call Waiting Deactivate: *70+number (before call) or *70 (during call)
Caller ID-Blocking per Call: Activate: *67+number to be blocked
Caller ID-Permanent Blocking Activate: *65 (Because of state laws, this
doesn't work in the state of Florida)
Caller ID-Permanent Unblocking: Activate: *85
Caller ID-Unblock Per Call: Activate: *82+number to be unblocked
Emergency Roadside Assistance: Existing Roadside Assistance customers
should dial #TOW (#869) on your wireless phone or 877-387-8766 from any
phone.
No Answer Transfer (Call Diversion): Activate: *71+number Deactivate:
*710 or *73
Voicemail Activate: *78; Deactivate: *780. (To retrieve voice mail,
customer must dial their 10-digit mobile number. Press * when you hear
greeting then follow voice mail prompts.)

One question: Does deactivating VM (*780) reset the OGM and password,
or does it simply temporarily turn it off, like turning your old
fashioned answering machine off?

In my area, it just turns it off. When you send a *78 to reactivate
your VM, your greeting is still there. There are some differences in
VM between different Alltel markets, but I have never heard of *780
wiping out your greeting.
