I had the same question about a year ago, and I called CS, and they told
me that M-t-M minutes worked in the entire national freedom network,
assuming that is the plan you are on. I did confirm this by writing down
a M-t-M call while roaming on verizon, and it did get billed as mobile
to mobile.
If you are on the total freedom plan, it is only supposed to work in
your state, or whatever your local freedom coverage area looks like, I
believe.

I am on Total Freedon, in South Carolina. I am told, and bills have
confirmed, that the other party must be an Alltel customer and must be
in either the NC or SC "local Freedom" area ... which included small
parts of Augusta and Savannah.
I was also told that any single landline number may be considered as
M-t-M and that most make it their home or office number, depending on
circumstance. Calls to/from must have the mobile in the area.

When calling, you have to be in your "home area". For LF and TF that is
the same as the local Freedom calling area. For National Freedom you
have to be in the NF network, in other words, not roaming.
I do not believe it matters where the other Alltel mobile is when you
call it.
The Most Often Called Number (MOC) can be added to the account when you
have 2 or more lines on one account. It is a local area number that
acts like a mobile for billing purposes for the lines on that account
(for both making and receiving calls from the mobiles.).
