Gentleman,
I support a very large number of enterprise users most who use Cingular's
Pre-Paid Go service for data (email, SMS, MMS, etc). We are getting a large
number of PIN/PUK Code issues. I believe this is mainly due to the fact
that the users are inserting their SIM into their device prior to the SIM's
activation. One other thing we due is to change their PIN/Password to the
last four digits of their assigned mobile number. Could this be causing the
PUK Code security issue or is there something else going on that we are
missing? If anyone can assist we would be grateful.

You need the PUK to change the PIN. Without it, the SIM gets trashed and you
wind up needing a replacement. Call C/S and get the PUK1 and PUK2 for each
number before trying to change the PIN. A PIN does NOT lock the phone; it
locks the SIM.
Inserting the SIM before activation has nothing to do with it, AFAIK.
