I'm an idiot and I entered the wrong PUK code more than 3 times in a
row.  So I lost my SIM card; is it possible to get back the stored
phone numbers in it?

If you have Logomanager and a data cable you can back up the SIM (the
old one) on your computer and then with the new SIM just transfer the
data back to a SIM.

Another alternative is if your phone has internal memory just copy all
the information on the SIM to the phone's memory and then transfer it
back to the new SIM card.
