voicemail notification... what's the norm?

I've seen some heated discussion in another mobile forum about
notification of voicemail that is being held.  Some operators send an
SMS alerting you that you have voicemail and telling you to dial a
specific number to retrieve it.  The other method is that instead of
receiving an SMS the "tape reel" icon on your handset will show plus
an audible sound to let you know that voicemail is there for you.  The
tape reel icon is supposed to remove itself once you check your
voicemail mailbox.  Which system is the norm in Europe, Asia, the
Americas?

I don't think there is any norm. Here in Austria, all five networks
use SMS, not the icon, btw.

Here in France, 3 operators, 2 using SMS notification, 1 using icon
(self removing after mailbox checking).

Why not all networks use the icon is beyond my comprehension. Switching the
icon on/off is done by sending a special SMS so why not send that??
