I have an unlocked motorola V66 phone that I bought in the US, I want
to send it to a friend in Montreal who needs a cell phone. Where, in
Montreal, can she buy a sim card that will work in that phone? Does
she have to change the band frequency on the phone?

You can send it to your friend, but the prospects of it working are
very poor. It is rare phone indeed that is sold in the USA that isn't
locked to the service provider that the service is with.
In the highly remote event that is unlocked, almost all GSM service
Providers offer prepaid service, and a SIM is part of the start up
package.

the phone is UNLOCKED, i got the unlock code from T-mobile

Both Rogers and Fido operate on the same GSM bands used in the US.
If memory serves, Rogers uses 850 and 1900, Fido uses 1900 only.

In Canada Fido sticks to the Majors Cities Ie: Toronto, Montreal,
Ottawa, Calgary, Edmonton, Vancouver etc. and the major highways between
these cities.
Rogers on the other hand gives the user access to most other smaller
communities but there are a lot of out of the way places that they don't
cover.
So the bottom line is if your friend sticks to mainly to major cities
and Major Highways then Fido would work just fine. Other wise Rogers
To get a Rogers Sim he will need to contact Rogers Customer service
to get a Fido Sim he can order it off the fido.ca site looking for the
Activation kit.
There are also some MVNO's that will sell activation kits too but they
just resell the Rogers/Fido service.
BTW There is only one GSM network in Canada now it is Rogers but they
keep for billing purposes Fido brand name and Fido's old network
footprint for billing purposes for people who only need to use their
phone in the City and certain major Highways
