I have a Verizon LG VX4650 phone. I want to program this phone to use
with Tmobile service since my Tmobile phone is broken. I can get in to
service menu and access all other admin setting but not sure what to put
in there besides the MIN and how to activate the phone for it to work.
This phone has no SIM.

If anyone has experience in programing this phone, please share your
knowledge. Thanks in advance.

You can't.  They use different technologies- Verizon is CDMA and T-Mo
is GSM.  You might as well try to tune in an FM radio station on an
AM radio.

That should be your first clue that this can't work.

Just go to Target or WalMart and buy the cheapest T-Mo prepaid phone
they sell and insert your SIM into it- it'll start working immediately.
