How do I go about setting my Nokia 6630 back to factory defaults?

Where does one get these codes for different things like showing
firmware versions, serial numbers and stuff?

I've had a 6630 a couple of weeks and just got a 1 GB card yesterday.
Now there's a few ogg files playing with oggplay :)

*#0000# shows firware version/ date  and  *#06# shows imei ( seriel) number

Standard restet code for all Nokia Phones is the five digit code string of
1:2:3:4:5.  Will reset your phone no matter what model it is.

Nokia doen't put this in the book. But they told me it was for all Nokia models.
