What are the PIN, PUK, PIN2, PUK2?

Do T-mobile (USA) SIM cards have default PIN1/2 PUK1/2 codes or do I have to
call customer service and ask?

Pin1 - yes (number should be in manual)
Pin2 - no
Puk1 - that is code when tmobile will give to you when you lock your
phone using wrong Pin1 few times
Puk2 - same thing but for Pin2 which tmobile doesnt offer
