Does anyone have the info as to where to email a request to unlock a
tmobile phone?

No more email. Dial 611 from your handset and talk to Customer Care.

T-Mobile no longer has an unlock "email" address. If you want to get
a handset unlocked you have to go through regular support channels
i.e. you need to call customer care (and be sure they know you want
the SIM unlock code and not the PUK code for a locked out SIM.)

The other alternative is to use the customer comment/query web form on
the t-mobile.com web page.
