Most likely it has to be an original, and would have to match the UPC
on your receipt, and that can vary..
Just an FYI...

Yeah, they need that whole sticker shpeal that goes along with the UPC.
IIRC, there were three different "things" on the sticker that we had to mail
in for our 6100 rebates. It had the UPC, ESN, ... umm... I dunno... but
yeah, what that d00d said is right. Chances are, your copy won't work.
Good luck though, sorry to hear about your trashed box.

Thanks for the tip. Fortunately, the rebate is through the retailer, not
the manufacturer. The form actually specifies a photocopy of the receipt.
So, I thought if there was any specific info, like a serial number, I could
just Photoshop my phone's info onto the copy.

A store would probably be mroe picky about a rebate than the
manufacturer.
Good luck...

Thanks for the tip. Fortunately, this rebate is from the store, not the
manufacturer, so I think I have a pretty good shot if I can find one. The
rebate form specifies a photocopy of the UPC, not the original. The Verizon
store told me that the serial number is listed with the UPC #, but I figure
I can always Photoshop my serial # in. Of course, I have to find one first
:-}
